Creating Wellness Lifestyle Questionnaire Instructions

Welcome to Creating Wellness!
 
We are excited to have you join our growing number of Creating
Wellness members. These instructions will guide you through the
registration and log-in process for the MycreatingwellnessSpace
web site. 

Follow these 8 simple steps to create your private password and
begin your journey into Creating Wellness.

Step 1  Go to http://www.creatingwellness.com

Step 2  Click on Wellness Member Login located in the
           upper right hand section of the web site.

Step 3  Click on Create Account.

Step 4  Your personal Access Code – 53060306. This code is
            required in order for you to proceed. Enter height in inches
            i.e. 65 for 5’5”.  Click Next when you have completed
            filling in the fields provided.

Step 5  Enter a User Name, Password and Email Address which
           will be used to activate and logon to the Creating Wellness
           Space website.  Click on Create User. 

Step 6  Your My CreatingWellness Space account has been established.
           You’ll receive a notification email asking you to activate your 
           account, please click on this link.  (If you do not have a valid
           email address your Creating Wellness coach can assist in
           activating your account.)

Step 7  Write down the Username and Password you selected
           Username: _________________________
           Password: _________________________

Step 8  After you have activated your account and returned to the
           website to login in please select LifeStyle Questionaire
           and begin answering the questions given. The Questionnaire
           will take approximately 10-15 minutes to complete.  Please
           read each question carefully and answer the questions based
           on your current lifestyle.

Congratulations!  You have completed the Lifestyle Questionnaire.
You are now ready to complete the physical assessment at Juszczyk
Chiropractic.
